CDS Alerts

Clinical Decision Support (CDS) alerts are used in conjunction with clinical monitoring to meet the Clinical Decision Support Rule objective.

The alerts listed below are based on Quality Measures with the goal of assisting providers to meet those measures during the patient’s stay. Because of this, it is suggested that sites choose the CDS Alerts based on their patient population and to use the “Constrain created time to today” options since the intention of the CDS alerts is to alert providers during “real time interactions” with the patient. Each alert the provider is interested in receiving should be subscribed to independently.

Hospital CDS Alerts

•NQF 0022 - Use of High-Risk medications in the Elderly - CDSAlertsID0156

oFilters the subscription to see alerts to the provider that indicates the patient is 66 years of age or older and has a home medication from the High Risk Medications for the Elderly Grouping Value Set. Alert will trigger when a new high risk home medication is added, when Medication Reconciliation is first accessed on a patient that has a high risk medication carried forward from a previous visit, and when a high risk medication is imported through Clinical Reconciliation.

•NQF 371 Venous Thromboembolism Prophylaxis - CDSAlertsID0108

oFilters the subscription to see alerts to the provider that a patient has been identified as having a moderate to very high risk of VTE, but no prophylaxis has been ordered.

•NQF 372 Medication Review - CDSAlertsID0190

oFilters the subscription to see alerts to the provider that an Admission Medication Reconciliation has not been performed on this patient for the current visit.

•NQF 373 Venous Thromboembolism Patients with Anticoagulation Overlap Therapy - CDSAlertsID0073

oFilters the subscription to see alerts to the provider when overlap therapy of warfarin and parenteral therapy has not been ordered for this VTE patient.

•NQF 374 Venous Thromboembolism Receiving Unfractionated Heparin - CDSAlertsID0109

oFilters the subscription to see alerts to the provider that it has not been documented that this patient's heparin treatment is being adjusted by a protocol.

•NQF 375 Venous Thromboembolism Discharge Instructions - CDSAlertsID0110

oFilters the subscription to see alerts to the provider that educational materials have not been provided to VTE patient or caregiver at discharge.

•NQF 376 Date of Birth Not Addressed - CDSAlertsID0114

oFilters the subscription to see alerts to the provider when the patient’s date of birth has not been entered within the system.

•NQF 435 Discharged on Antithrombotic Therapy - CDSAlertsID0104

oFilters the subscription to see alerts to the provider when Antithrombotic therapy has not been prescribed for an ischemic stroke patient.

•NQF 436 Anticoagulation Therapy for Atrial Fibrillation/Flutter - CDSAlertsID0071

oFilters the subscription to see alerts to the provider when Anticoagulation therapy has not been prescribed for a patient with atrial fibrillation/flutter.

oFilters the subscription to see alerts to the provider when Thrombolytic therapy has not been initiated for acute ischemic stroke, and there is no documentation of a medical reason for patient to not be on Thrombolytic therapy.

oFilters the subscription to see alerts to the provider when Antithrombotic therapy has not been ordered, and there is no documentation of a medical reason for the patient to not be on Antithrombotic therapy.

oFilters the subscription to see alerts to the provider when Statin medication was not prescribed for ischemic stroke patient.

•NQF 440 Stroke Education - CDSAlertsID0107

oFilters the subscription to see alerts to the provider that educational materials have not been provided to a stroke patient or caregiver at discharge.

•NQF 441 Assessed for Rehabilitation - CDSAlertsID0102

oFilters the subscription to see alerts to the provider when a rehab assessment has not been ordered for this stroke patient.

•NQF 497 Psychiatric/Mental Health Patient Documentation - CDSAlertsID0111

oFilters the subscription to see alerts to the provider to ensure proper documentation for a patient admitted from emergency department to inpatient setting who has been diagnosed with a mental health problem.

Clinic CDS Alerts

•NQF 028 Tobacco Use: Screening and Cessation Intervention - CDSAlertsID0138

oFilters the subscription to see alerts to the provider that a patient's smoking status indicates use of tobacco products, and there is no documentation that tobacco cessation counseling was performed at least once within the last two years.

•NQF 024 Weight Assessment with Nutrition and Physical Activity Counseling - CDSAlertsID0155

oFilters the subscription to see alerts to the provider when their patient doesn't have height or weight recorded.

•NQF 069 Treatment for Children with Upper Respiratory Infection - CDSAlertsID0154

oFilters the subscription to see alerts to the provider when their patient has been diagnosed with Upper Respiratory Infection (URI) to evaluate if antibiotic therapy is necessary.

•NQF 002 Appropriate Testing for Pharyngitis - CDSAlertsID146

oFilters the subscription to see alerts to the provider when their patient was diagnosed with pharyngitis without a group A streptococcus laboratory test.

•NQF 018 Blood Pressure Assessment Has Not Been Documented - CDSAlertsID0165

oFilters the subscription to see alerts to the provider when their patient's blood pressure assessment has not been documented in the last 6 months. This alert will also trigger if the following applies: patient has a diagnosis on the Problem List from Essential Hypertension Value Set, patient does not have a diagnosis from the Pregnancy Value Set, the Chronic Kidney Disease - Stage 5 Value Set, or the End Stage Renal Disease Value Set.

•NQF 059 Comprehensive Diabetes Care- Hemoglobin a1c Control - CDSAlertsID0122

oFilters the subscription to see alerts to the provider when a patient has a diagnosis of diabetes and has not had an HbA1c ordered recently.

•NQF 608 Prenatal Screening for HBV - CDSAlertsID0158

oFilters the subscription to see alerts to the provider when a patient is pregnant and has not been screened for HBV infection.

•NQF 036 Use of Appropriate medications for Asthma - CDSAlertsID0126

oFilters the subscription to see alerts to the provider when a patient has a diagnosis of asthma without any asthma medication therapy ordered.

•NQF 421 Body Mass Index Screening and Follow-up - CDSAlertsID069

oFilters the subscription to see alerts to the provider when a patient does not have height or weight recorded within the past six months.
