The Check Boxes will allow multiple answers to be selected for the same question.

 

1.The question “General Appearance” is an example of a Check Box question. Select all answers that apply.

 

2.To deselect an answer, select the answer again and the check mark will disappear. For example, if “No Acute Distress” was selected in error, select the answer a second time and the check mark will disappear.

 

 

Select Charts > Select Patient > Electronic Forms > Check Box
