The disablement option is set up when the Electronic Form is created. When the answer is selected, it will not allow any other answer for the question to be selected. An example would be the question “History: With the Answers of Patient, Family, Social, None” and the answer “None” will disable the remaining choices.

1.After selecting the Electronic Form, select the answer None for the History question.

2.The remaining answers are inaccessible unless the answer None is unchecked.

3.If the answer None was selected in error, select None again and the answers that were inaccessible are now accessible.
