Allergies, reactions, priority or severity may be setup to display within an Electronic Form as VIEW ONLY. This will allow allergies entered within the Allergies application, or within the Allergy section in Clinical Information, to display on a form. To add allergy information from an electronic form to the patient's account, the Allergy widget must be accessed and all allergy data should be entered within the Allergy Application. Please see Applications to set up the Allergy widget.
NOTE: Electronic Forms built prior to Version 20 with the allergy auto-populate setup will still display the allergy information on the patient's chart, but in order to access the new allergy reactions table for PI3, new allergy data must be entered in the Allergy application via the Allergy widget. When new allergies are added via the Allergy widget, and View Only Allergy fields are on the form, the Electronic Form must be refreshed by saving/exiting and re-entering the form, or moving to another page and re-entering the page with the allergy fields, in order for the newly entered allergy information to pull to the form.
To setup View Only Allergies, Reactions, Severity and Priority on a form, follow the steps below:
NOTE: When building View Only Allergy text boxes, it is recommended to build each individual allergy on its own individual line when pulling in the reactions, severity or priority with the allergy.
Allergy Setup:
1.Select New Control.
2.Select Text Box.
3.Select Start New Line on the Label tab. If this is used in a mini-column setup, the label does not have to be entered.
3.Select the Database tab to associate a database code to the text box.
4.Select the Magnifying Glass and choose the database code *PATALLER to pull in the View Only allergy. The notification "Automatically changed your answer size from 00 to 35" will display, select OK. This is notifying the user that it is adjusting the answer width on the answer table to the appropriate width needed for the database code. The data path does not need to be addressed since the database code selected is hard coded to be Upload Only.
5.Select the Answer tab. The width was automatically filled by adding the database code prior to accessing the answer tab, so that will not need to be modified. In the Title field, enter "Allergy #," where the "#" is the number that the allergy will be. For example, the first text box with the *PATALLER database code will be Allergy 1.
6.Select Save and Exit.
Reaction Setup:
1.Select New Control.
2.Select Text Box. If this is used in a mini-column setup, a label does not have to be entered on the Label tab.
3.Select the Database tab to associate a database code to the text box.
4.Select the Magnifying Glass and choose the database code *PATALREAC to pull in the View Only allergy reaction. The notification "Automatically changed your answer size from 00 to 40" will display, select OK. This is notifying the user that it is adjusting the answer width on the answer table to the appropriate width needed for the database code. The data path does not need to be addressed since the database code selected is hard coded to be Upload Only.
5.Select the Answer tab. The width was automatically filled by adding the database code prior to accessing the answer tab, so that will not need to be modified. In the Title field enter "Reaction #," where the "#" is the number that the allergy reaction will be. For example, the first reaction text box following the first allergy would be Reaction 1.
6.Select Save and Exit.
Severity Setup:
1.Select New Control.
2.Select Text Box. If this is used in a mini-column setup, a label does not have to be entered on the Label tab.
3.Select the Database tab to associate a database code to the text box.
4.Select the Magnifying Glass and choose the database code *PATALSEVR to pull in the View Only allergy reaction. The notification "Automatically changed your answer size from 00 to 20" will display, select OK. This is notifying the user that it is adjusting the answer width on the answer table to the appropriate width needed for the database code. The data path does not need to be addressed since the database code selected is hard coded to be Upload Only.

Priority Setup:
1.Select New Control.
2.Select Text Box. If this is used in a mini-column setup, a label does not have to be entered on the Label tab.
3.Select the Database tab to associate a database code to the text box.
4.Select the Magnifying Glass and choose the database code *PATALPRIR to pull in the View Only allergy priority. The notification "Automatically changed your answer size from 00 to 10" will display, select OK. This is notifying the user that it is adjusting the answer width on the answer table to the appropriate width needed for the database code. The data path does not need to be addressed since the database code selected is hard coded to be Upload Only.
5.Select the Answer tab. The width was automatically filled by adding the database code prior to accessing the answer tab, so that will not need to be modified. In the Title field enter "Priority #," where the "#" is the number that the allergy priority will be. For example, the first priority text box following the first allergy would be Priority 1.
6.Select Save and Exit.
Repeat the above setup for as many rows that are needed for VIEW ONLY allergies to display within the form.
NOTE: If a patient has more allergies than View Only allergy fields built on the Electronic Form, the following warning will display "WARNING: There are not enough fields setup to display every allergy for this patient. This may cause unexpected behavior." The user should then review the allergies via the Allergy Application or from the Patient ID panel for the full listing of the patient's allergies.
