1.To enter a new problem, begin typing the problem description in the Add Problem search box. From the search results list that appears, select the appropriate problem.
NOTE: If the searched-for problem already exists on the patient's chart, the system will highlight the existing problem on the card, to draw attention to that problem and prevent duplicate entry of it.
2.The Diagnosis Refinement tool will display to assist in selecting a specific ICD code for the problem. Select Refine to narrow down the list of ICD codes. Select the Diagnosis Attributes that apply, and then select the appropriate ICD code from the Refinement Options list. Then select Save to display the ICD code.
NOTES:
•If a problem has not been refined to a specific ICD code, it will display in yellow and may be selected to display the Refinement tool just like when a new problem is added to allow the user to refine the ICD Code.
•If the problem is refinable but maps to more than one possible SNOMED code, an asterisk appears next to the option in the Refinement Options list. Selecting an option marked with an asterisk opens an additional section listing the possible codes. Select the appropriate code and select Save.
3.Use the drop-down to select the status of the problem (Active, Historic, Provisional, or Recurrence).
NOTE: When a problem's status is changed to Historic, the Resolved date will auto fill with the date the problem the status was changed. If the Historic status of a problem is changed to Active, the Resolved date will automatically be removed. If the status is Recurrence, the problem will be included in the Active section of the list.
4.Select Save to add the new problem to the card.
