Problem Management

The Problem Management card displays the patient’s problems. From this card, you can add new problems and manage existing problems that may be pertinent to the current visit. A table within the card will display each problem along with the ICD Code and the Status of the problem. These fields can be edited as needed.

The card is divided into four sections. An expand/collapse icon (chevron) displays to the left of each of these sections, allowing you to expand or collapse the rows within the section.

 

Treating This Visit will be empty for each new visit. Newly entered problems with a status of Active or Provisional will automatically display within this section.

oTo add an existing problem from a previous visit to this visit, select the treat_problem_icon Treat Problem icon. Selecting the Treat Problem icon on a historical problem will automatically change the status of the problem to Active.

oDeselecting the Treat This Visit icon will display the following prompt: "All links to Notes documentation and encounter history will be lost. Do you wish to continue?" Select Yes to remove the problem from within Treating This Visit or select No to keep the problem within the Treating This Visit section.

oIf a resolved date is entered during the visit for a problem listed within the Treating This Visit section, it will remain within that section for that visit. The resolved problem will move to Historical on the next encounter.

oIf the status of a problem that is listed within the Treating This Visit section is changed from Active or Provisional to Historical, that problem will remain within that section for that visit. The historical problem will move to the Historical section on the next encounter.

Active will display any problems within the problem list that are currently active.

Historical will display any problems that are currently marked as resolved within the problem list.

Archive will display any problem flagged as archived. Problems flagged as archived are user specific and also specific to Patient Data Console. They will not display within the Problem List application.

NOTE: To move a Historical problem to the Archive section, select the Actions icon actions_icon to the right of the problem and select Archive. Only problems with a status of Historical can be archived. To restore an archived problem, select the restore_icon_pmRestore icon next to the problem in the Archive section.

 

 

To View and Edit Problems

Hover over a problem to display the Edit edit_procedure_icon icon. Select this icon to edit the problem as needed.

 

To change the order in which problems display within a section (i.e., to indicate the priority of the problems), drag them by the move icon (the horizontal lines) to the left of the problem. The priority order each user sets is specific to that user and to Patient Data Console. It will not display within the Problem List application.  

 

To view more details about a problem, select the expand (chevron) to the left of the problem. The following information will display. Editable fields can be modified as needed.

oSNOMED & ICD10: Selecting the expand (chevron) icon will display the SNOMED and ICD10 code(s) and the description associated with the problem.

oOnset Date: Allows the onset date for the problem to be entered. If an onset date is available within the problem list, it will display within the field. To enter a date, select the date format: MM/DD/YYYY HH;MM, MM/DD/YYYY,  MM/YYYY, or YYYY. Then enter the date in the format selected. The default selection is MM/DD/YYYY. The onset date may be edited or removed at any time.

oResolved Date: Allows the resolved date for the problem to be entered. If a resolved date is available within the problem list, it will display within the field. To enter a date, select the date format: MM/DD/YYYY HH;MM, MM/DD/YYYY, MM/YYYY, or YYYY. Then enter the date in the format selected. The default selection is MM/DD/YYYY. When a problem's status is changed to Historic, the Resolved date will auto fill with the date the problem the status was changed. If the Historic status of a problem is changed to Active, then the Resolved date will automatically be removed. The resolved date may be edited or removed at any time.

oComments: Allows additional information about the problem to be entered.

oPrevious Activity: This area will display any encounter where the problem was treated within a previous visit. The information that will display is the Encounter/Visit the problem was treated for, the Admission date for that visit, and the Discharge date.

To delete a problem, select the Actions actions_icon icon and choose Delete Problem. For Historical problems, you can choose either Delete Problem or Archive.

oSelecting Delete Problem marks the problem as Entered in Error. Once selected, the prompt "If you click Yes, (insert problem name) will be removed. Continue?" will display. Selecting Yes will remove the problem, and selecting No will make no changes to the problem.

oSelecting Archive will move a historic problem to the archive section so that it no longer displays within the viewable space of the card.

To change the default description shown for problems on the card (ICD Code vs. SNOMED Description), select the Filter (cog) icon and make the desired selection. The selected primary description will display on the face of the card. To view the secondary description, select the expand (chevron) icon. The secondary description will display above the Onset Date and Resolved Date for the selected problem.

 

NOTE: A valid resolved or onset date must be entered in order for the field to save. A valid onset date or resolved date may not be a future date or a date prior to the patients date of birth.
