A Transfer Form flow chart can include predefined transfer statements for patient consent, request, refusal, and predefined facility names and addresses that patients are commonly referred to. When these questions are selected on the flow chart, the statement and facility name and address that the patient is being transferred to will be included on the Transfer Form report along with a signature area.
