To create a new date and time column for documentation, select a question on the flow chart. The question/answer box displays. Select the appropriate answer(s) and select Next to save the information and advance to the next question. Continue answering questions and selecting Next to chart in the same date and time column. Once charting is complete, select Update. A date and time column will display on the flow chart, along with the answer text for the questions that were addressed.

 

To chart answers in an existing date and time column, select the question cell in that date and time column.The question/answer box displays. Select the appropriate answer(s) and select Next to save the information and advance to the next question. Continue answering questions and selecting Next for answers to be charted in the same date and time column. Once charting is complete, select Update.
