The Setup options are as follows:

 

Title

Usage: Enter the title of the new Electronic Form

Options: A Title with a maximum of 50 characters may be entered

Default: Blank

 

Category

Usage: Uploads information that is already existing in TruBridge EHR or downloads information as it is answered on the Electronic Form

Options: Patient, Human Resources

Default: Patient

 

OR Form

Usage: If checked, a new control will display on the toolbar for this form which will allow the user to designate the form to a specific OR visit. When a form designated as an OR form is initially opened, the value indicating the visit number will default to 1, then progress numerically as additional OR forms are opened on that same account. At any time, if the visit number that is displaying is incorrect, it may be manually altered by selecting the "+" or "-" options associated with this field. Multiple forms may be designated as OR Forms and therefore be generated under the same OR visit. EX: Preop, intra op and post op forms may be grouped under a single visit. A Database code (PATORVISIT) is set up on the Electronic Form. The Database code should be set up on a Text Box control and be set to upload and download. See Text Box section for instructions regarding setting up database codes on the control. When an Adhoc Report that contains the information from this form is being generated, the field "Separate by OR visit" should be checked on the report generation screen.

Options: To designate an Electronic Form as an OR Form so that multiple visits to the OR during the same stay may be separated if the documentation is queried through the Adhoc Reporting system.

Default: Blank

 

NOTE: The option to separate the documentation is currently only viewable in pdf format. The ability to separate the documentation for viewing in Excel/Access is being pursued.

 

Subcategory

Usage: When selected, the forms will display on the selection screen by subcategory.

Options: This field is user defined. See Tables for instructions on setting up a new Subcategory.

Default: Blank

 

Number of Pages

Usage: Displays the total number of pages on the form. Number of pages, must match the number of Page Controls on the form setup.

Options: 01-99

Default: 01

 

Columns

Usage: Select the number of columns for the new Electronic Form. This option selected will allow a form to be split into two columns by enabling the Start Column and End Column options on the Layout Control.

Options: One, Two.

Default: One

 

Disablement Result

Usage: When using a one column form, this field gives the option to disable or hide questions from view.

Options: Disable, or Hide a question(s) on a one column Electronic Form only.

Default: Disable

 

Initial Column Widths

Usage: Sets the column width for the Electronic Form if two columns will be used.

Options: The columns may be set to any width as long as the total does not exceed 744.

Default: The first and second column width default is 372 but the width may be changed as long as the total of the columns does not exceed 744.        

 

Text Box Style

Usage: Determines how the text box, drop-down box and lookup box will appear on the form.

Options: Flat, normal, and underline. The Normal option will display a box on the screen for answers set up with boxes. Underline option will display underlines on the screen for answers set up with boxes.

Default: Normal

 

Default Font Type

Usage: Sets the default font for the Electronic Form

Options: Arial, Courier New, and Times New Roman

Default: Arial

 

Default Font Size

Usage: Sets the default font size for the Electronic Form

Options: 6, 8, 9, 10, 11, 12, 13, 14, 15, 16, 17, 18, 19, 20, 22, 24

Default: 8

 

Default Background Color

Usage: Sets the default background color for the Electronic Form

Options: There are 3 pages to choose from totaling 144 colors

Default: White

 

Control Increments

Usage: Sets the spacing between entries on the Electronic Form

Options: 001-999

Default: 020

 

Level of Care option, Service Type, and Exam Type are no longer used.
