Three reports are available: Unlocked Electronic Forms, Abnormal Exit Log, and Override Report. The Unlocked Forms report shows all forms that have not been locked on patient accounts. The abnormal Exit Log will show a list of forms, patient account numbers with dates and times the user closed out of the form on the patient without saving the form properly. The Override Report will show entries captured when a form is accessed that is currently in use.

 

1.From the Electronic Form Selection screen, select Reports.

 

2.Select Unlocked Forms then choose a printer.

3.Choose the Filter Type: By Patient or By Physician.

4.If option “By Patient” is chosen, select from the following three options:

 

All Accounts: This option prints all patients that were admitted through the applicable date range that have unlocked forms. Enter beginning Admit date and End Date or hit Print for all patients.

 

By Account Range: Enter an account number in the Begin range and End Range fields. This report displays patients with unlocked forms who have an account number that is applicable to the range entered.

 

By MR  Number Range: Enter a MR Number in the Begin and End Range fields. This report displays patients with unlocked forms who have a Medical Record number that is applicable to the range entered.

 

By Admit Date Range: Select a Beginning and Ending date range. This report displays patients with unlocked forms within the applicable range entered.

 

5.If ‘By Physician’ is chosen, the report will display all unlocked forms on patients which have the selected physician listed as either the admitting, second, family or consulting physician.

 

Choose the physician by selecting the magnifying glass and selecting the physician from the list.

 

Limit the report by selecting a Begin and End Date range.

 

Select Print to display or print the report.

 

 

Unlocked Electronic Forms Report generates the following information:

 

Title of Report: “Unlocked Electronic Forms” - prints at the top center of report.

 

Generation Parameters: Includes Account # range, MR # range, Physician name or group centered just below the title of Report.

 

Report Date: System date for the date the report was generated

 

Patient Name

 

Medical Record # or Account # (depending on how report was generated)

 

Title of Electronic Form

 

Created On: Date Electronic Form was created

 

Last Updated On/By: Date Electronic Form was last updated on/by.

 

 

Abnormal Exit Log

 

A report may be generated to view a list of occurrences of an abnormal exit from the form which includes: exiting the form by selecting the red "X," pressing key combinations CTRL+W or ALT+F4, or by pressing F5 and selecting the Cancel/Exit icon. The report may be printed from the Electronic Forms Selection Screen in maintenance.

 

From the Hospital Base Menu, Master Selection, Business Office Tables, Business Office Tables Maintenance, Electronic Forms, Reports, Abnormal Exit Log.

 

 

The log will give the following information:

 

Date and Time action occurred

 

Action (will always say Abnormal Exit)

 

Data Loss

 

Logname

 

Employee signon

 

Electronic Form code and sequence

 

Patient account number

 

IP address of the device that was used

 

Home directory that the device is logged into

Override Report

 

TruBridge EHR has been enhanced to give a prompt whenever a form is accessed that is currently in use. The prompt will read: "This form cannot be accessed at this time because it is being edited by....:" the prompt will give the employee name, ID, and date the form was accessed. There will be three options to choose from once the prompt appears - Retry, Override, Cancel.  If Override is selected, another prompt will display stating this will add a record in the Override Report.

 

The report may be printed showing the following information and the report information should sort by Patient name:

 

Title of Report: "Electronic Forms Override Report" - should print at top center of the report.

 

Generation Parameters: Includes Account # range, MR # range, Employee number range, Physician name or group - should print centered just below title of Report.

 

Report Date: Have system print the date the report was generated - should print centered just below generation parameters.

 

Patient Name (Report should sort using this field)

 

Medical Record # or Account # (depending on how report was generated)

 

Title of Electronic Form

 

Created On - pulls from the Electronic Forms selection screen

 

Override By/On - pulls from the record created when an employee chose the override function.

 

NOTE: There are instances when the "Form in use" prompt will display and it may not be open on another device. These include situations where the form was not previously closed properly, including power outage, battery failure, control/alt/delete, etc. This prompt will display for up to 14 hours at which point the "Form in use" lock will be automatically removed or if override is selected.
