Medication Reconciliation in CPOE

Medication Reconciliation will allow the reconciliation of a patient's home medications upon admission. Once an admission reconciliation has been completed, any home medications that are continued will generate active medication orders in Order Entry.

 

Once the user has performed an admission reconciliation, if any home medications were continued/modified, it will launch the user to the Pending Order Review screen from the Medication Reconciliation option. The items will appear in the order in which they were addressed. If any therapeutic substitutions were selected, these will also appear under Pending Orders. The orders will also be available on the Search Options screen in the Pending Orders portion of the screen.

 

If any active medications are modified and the exact same medication is selected (same or different strength), Duplicate Therapy Clinical Monitoring checks should not run against the medication that is being discontinued.

The Med Rec orders in the pending list should show the medication name along with the order detail information (dose, unit, route, and frequency) for each medication.

 

Any medications that have Clinical Monitoring results will display with the icons. The user will be able to address these by accessing the Address Clinical Monitoring option.

 

Additional orders may be placed after the user has been directed to Order Entry. Once the user has addressed all items and signed the orders, they will be returned to the Medication Reconciliation screen where any newly created medications will appear in the Active column.
