The Generate QR Code option is used to generate a SMART® Health Card with a QR code that contains the patient's immunization information. The card can be provided to the patient and shared with those they trust.

 

1.To generate a SMART® Health Card with a QR code, select Generate QR Code.

2.Select the check box next to Health Card you want to generate. Options are Immunization Card or COVID-19 Immunization Card.

 

3.Select Generate. The SMART® Health Card displays.

4.To print the card, select the Print icon.
