A Physical Assessment flow chart will automatically default to Initial unless it is set to default to Shift in the the Flowchart Setup. Documentation on an Initial Physical Assessment can be viewed on the Initial Physical Assessment report, and documentation on a Shift Physical Assessment can be viewed on the Patient Progress Notes report.

 

Physical Assessment flow charts may include special questions that display a unisex body diagram for documenting pain, wound, and decubitus locations, and a predefined Pain Scale.
