Recording Volume and Frequency

1.Select New Output on the flow chart. The Enter New Output box displays.

2.The Date/Time fields default to the current date and time. Modify these fields, if necessary.

 

3.When the box is initially accessed, Volume Entry displays by default in the first field. If charting frequency, use the drop-down and select Frequency Entry.

 

4.In the Enter Output Site field, use the drop-down to select the output site. If the output site is not listed, select OTHER from the drop-down list. On the Other Description Entry box, enter the output site description and select OK.

 

5.In the Volume or Frequency field, use the number pad to enter the output volume or frequency.

 

NOTE: To record zero as the output volume, select None on the number pad.

 

6.In the Drain Level field, use the number pad to enter the drain level, if applicable.

 

NOTE: When recording the frequency of stools, the size must be documented.

 

7.Select Update to save the information and return to the flow chart or Cancel to close the box without saving and return to the flow chart. When Update is selected, the output selection will display on the flow chart for future documentation.
