1.Select VS Combo on the flow chart. The Enter VS Combo box displays.

2.The Date/Time fields default to the current date and time. Modify these fields, if necessary.

 

3.In the Temperature field, use the number pad to enter the temperature.

 

4.In the Choose Site field, use the drop-down to select the temperature site.

 

5.In the Pulse field, use the number pad to enter the pulse rate.

 

6.In the Choose Site field, use the drop-down and select the pulse site.

 

7.In the Respiration field, use the number pad to enter the respiration rate.

 

8.To record blood pressure:

 

•In the Systolic field, use the number pad to enter the systolic pressure.

 

•In the Diastolic field, use the number pad to enter the diastolic pressure.

 

•In the Choose Posture field, use the drop-down to select the patient's posture.

 

•In the Choose Site field, use the drop-down to select the blood pressure site.

 

9.In the O2 Saturation field, use the number pad to enter the O2 saturation level.

 

10. Select the Pain f/s check box to have the vital sign information print to the Pain Flowsheet.

 

11.Select Next to save the information and advance to the next question, Update to save the information and return to the flow chart, Cancel to clear the entry and return to the flow chart, or Prev to save the information and go to the previous question.

 

NOTE: Not all vitals in the combo box need to be addressed. However, the vitals that are addressed must have all the information such as site and posture selected to save the information.
