Multidisciplinary staff can document an Initial Interview, Physical Assessment, and Activities via flow charts. Please refer to the appropriate sections in this guide on flow chart usage. Multidisciplinary flow charts can be viewed and printed separately from nursing documentation for the Initial Interview, Initial Physical Assessment, Activities, and Patient Progress Notes.
