To document follow-up care when ordered for a discharge patient after admission to the emergency department, a check box is used with a hard-coded database code attached. The answer may then be pulled to an ad hoc report or populate the form from a flowchart or to a flowchart question.

 

To create the check box:

 

1.Select the Electronic Form in the maintenance screen.

 

2.Select New Control and choose Check box.

 

3.Label the control i.e. "Followup Care ordered for a discharge patient after admission to ED."

 

4.Select the Database Tab then magnifying glass. Select MUEDFUCARE database code.

 

5.Select upload and download and this will pull the answer from a POC Flowchart or to a flowchart question if it is entered on the form first.

 

 

Select Electronic Form > New Control > Check box > Followup Care Ordered for Discharge
